Published by the organization “Aging with Dignity”, Five Wishes is a living will that addresses your personal, emotional and spiritual needs as well as your medical wishes. This document helps start the important conversations with family members and your doctor about your end-of-life care wishes.
Five Wishes lets your family and doctors know:
- Who you want to make health care decisions for you when you can’t make them.
- The kind of medical treatment you want or don’t want.
- How comfortable you want to be.
- How you want people to treat you.
- What you want your loved ones to know.

This is an end-of-life planning kit. Included is a New Jersey-specific Living Will and other advance medical directives, plus a 16-page write-in booklet, with attractive illustrations by Edward Gorey, which includes space to record your wishes as to funeral and memorial services, information your survivors need to know, and a checklist of other important documents to store there (like military discharge papers). Life After Death – A Wellness Guide for the Bereaved by Lucille Rosetti
Lucille Rosetti created TheBereaved.org as a means of sharing tools to help people through the grief process. Having lost some of the people closest to her, she understands what it’s like, and how it can be an emotional roller coaster that doesn’t always seem to make sense.
